O instrumento de Pactua\u0026ccedil;\u0026atilde;o dever\u0026aacute; conter em seu objeto as metas mensais e anuais a serem alcan\u0026ccedil;adas, em rela\u0026ccedil;\u0026atilde;o aos indicadores de sa\u0026uacute;de, acordadas e firmadas no Pacto de Indicadores da Aten\u0026ccedil;\u0026atilde;o B\u0026aacute;sica.\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpLast\\\u0022\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EArt. 3\u0026ordm;.\u003C\/strong\u003E Fica obrigado o compartilhamento entre membros das equipes, bem como entre equipes o compromisso pactuado, as responsabilidades, o desempenho f\u0026iacute;sico e financeiro.\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003ESe\u0026ccedil;\u0026atilde;o II\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003EDa Produtividade\u003C\/p\u003E\r\n\u003Cp\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EArt. 4\u0026ordm;.\u003C\/strong\u003E Cabe \u0026agrave;s equipes de sa\u0026uacute;de da fam\u0026iacute;lia buscarem a viabiliza\u0026ccedil;\u0026atilde;o da organiza\u0026ccedil;\u0026atilde;o assistencial e o fortalecimento da Aten\u0026ccedil;\u0026atilde;o B\u0026aacute;sica, seguindo as diretrizes, no \u0026acirc;mbito do territ\u0026oacute;rio municipal de forma homog\u0026ecirc;nea.\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EPar\u0026aacute;grafo \u0026Uacute;nico.\u003C\/strong\u003E Para o cumprimento do dever descrito neste artigo dever\u0026atilde;o ser praticadas a\u0026ccedil;\u0026otilde;es de sa\u0026uacute;de, de abrang\u0026ecirc;ncia individual ou coletiva, que visem \u0026agrave; consecu\u0026ccedil;\u0026atilde;o da melhoria dos servi\u0026ccedil;os prestados seja com a preven\u0026ccedil;\u0026atilde;o de agravos, diagn\u0026oacute;stico, tratamento, reabilita\u0026ccedil;\u0026atilde;o e manuten\u0026ccedil;\u0026atilde;o da sa\u0026uacute;de.\u003C\/p\u003E\r\n\u003Cp\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EArt. 5\u0026ordm;.\u003C\/strong\u003E Trimestralmente, o respons\u0026aacute;vel imediato pelo gerenciamento das equipes de sa\u0026uacute;de da fam\u0026iacute;lia fica obrigado a gerar relat\u0026oacute;rio gerencial de produtividade, especificando \u0026ndash; de forma mensal \u0026ndash; as a\u0026ccedil;\u0026otilde;es realizadas no cumprimento das metas.\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003ESe\u0026ccedil;\u0026atilde;o III\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003EDo Apoio Log\u0026iacute;stico\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003E\u0026nbsp;\u003C\/strong\u003E\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EArt. 6\u0026ordm;.\u003C\/strong\u003E A Secretaria Municipal da Sa\u0026uacute;de garantir\u0026aacute; todo o apoio log\u0026iacute;stico para o desenvolvimento das a\u0026ccedil;\u0026otilde;es estrat\u0026eacute;gicas desenvolvidas pelas equipes de sa\u0026uacute;de da fam\u0026iacute;lia\u003Cstrong\u003E.\u003C\/strong\u003E\u003C\/p\u003E\r\n\u003Cp\u003E\u003Cstrong\u003EPar\u0026aacute;grafo \u0026Uacute;nico.\u003C\/strong\u003E Para garantia do apoio log\u0026iacute;stico, dever\u0026aacute; ser realizado planejamento pr\u0026eacute;vio e comunica\u0026ccedil;\u0026atilde;o, com anteced\u0026ecirc;ncia m\u0026iacute;nima de 30 (trinta) dias, ao superior imediato respons\u0026aacute;vel pelas equipes, devendo ser encaminhado para o Secret\u0026aacute;rio da Sa\u0026uacute;de os casos em que seja exigida a realiza\u0026ccedil;\u0026atilde;o de processo licitat\u0026oacute;rio.\u003C\/p\u003E\r\n\u003Cp align=\\\u0022center\\\u0022\u003E\u0026nbsp;\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpFirst\\\u0022 align=\\\u0022center\\\u0022\u003ECAP\u0026Iacute;TULO III\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022 align=\\\u0022center\\\u0022\u003E\u003Cspan\u003EDO RATEIO DOS REPASSES PARA as Equipes\u003C\/span\u003E\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022 align=\\\u0022center\\\u0022\u003E\u003Cspan\u003E\u0026nbsp;\u003C\/span\u003E\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022\u003E\u003Cstrong\u003EArt. 7\u0026ordm;.\u003C\/strong\u003E Os recursos federais ser\u0026atilde;o rateados de acordo com a produtividade das equipes, trimestralmente, ap\u0026oacute;s o repasse realizado pelo Minist\u0026eacute;rio da Sa\u0026uacute;de, em duas fases.\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022\u003E\u003Cstrong\u003E\u0026sect;1\u0026ordm;.\u003C\/strong\u003E No processo de ades\u0026atilde;o, o valor repassado ser\u0026aacute; integralmente rateado.\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022\u003E\u003Cstrong\u003E\u0026sect;2\u0026ordm;.\u003C\/strong\u003E Ap\u0026oacute;s a qualifica\u0026ccedil;\u0026atilde;o das equipes, somente ser\u0026aacute; rateado 50% (cinquenta por cento) do valor repassado.\u003C\/p\u003E\r\n\u003Cp class=\\\u0022MsoNormalCxSpMiddle\\\u0022\u003E\u003Cstrong\u003E\u0026sect;3\u0026ordm;.\u003C\/strong\u003E O rateio dos valores ser\u0026aacute; pol\u0026iacute;tica adotada pelo Minist\u0026eacute;rio da Sa\u0026uacute;de, bem como, por ser somente reparti\u0026ccedil;\u0026atilde;o de repasses de acordo com o desenvolvimento das equipes, n\u0026atilde;o ser\u0026aacute;
